This facility is a long-term care and skilled-nursing facility located in Council Bluffs, IA.

Responsibilities Guides responsible parties in completion of Iowa Medicaid applications.
Assists patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care by performing the following duties.
Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem.
Plan, coordinate, manage and implement support packages to help clients deal with difficulties and overcome dependencies Interview residents and assess their current condition, needs, strengths and weaknesses via PHQ-9 & BIMS assessments Address each case as a unit and set tailored measurable goals Monitor and evaluate resident's progress and modify treatment plans accordingly Orients residents and families to the facility, its services, its service limitations, and resident's rights.
Completes mandatory documentation within timelines established by applicable regulations.
Mediates issues that arise among residents, families, and staff - is the facility grievance officer.
Offer information and counseling on the best course of action Maintain accurate records and report on resident's status, acting as a liaison between facility clinical staff and physician services.
Act as a key-worker and cooperate with multidisciplinary teams for resident care plans and care plan conferences Refer residents to community services as needed Skills Proven work experience as a social worker Working knowledge of social theories and practices Reliable & Have great attention to detail Social perceptiveness and empathy Ability to build and maintain professional helping relationships Ability to relate and communicate with diverse population and groups Resilience along with ability to assess situations Willing to submit to background checks Preferred, BS degree in social work (BSW), psychology or sociology Job Type:
