Are you a Social Worker (MSW) looking to work for an amazing organization by serving others and connecting with your co-workers? Immanuel is seeking a Social Worker (MSW) to work at Pathways Southwest Iowa located at 1702 North 16th Street, Council Bluffs, IA 51501.
Schedule is Monday though Friday, 8am to 5pm.
Pay will be based upon experience, certifications, skills and education.

What You Will Do Plans, organizes and implements social services to Immanuel Pathways participants and families.
Responsibilities include but are not limited to:
assessment, treatment, teaching and counseling to participant, caregiver or other appropriate representatives.
Social work interventions could include individual participant contacts; appropriate collateral contacts; participant and family education; assessment and counseling; provision of resources; ongoing case management; advocacy to ensure participant and caregiver needs are met and addressed; and disenrollment procedures.
Skills & Requirements Masters Degree from an accredited school of social work required.
Licensed in the state of Iowa.
Two (2) years of experience working on a multidisciplinary team in a hospital, nursing home or community-based setting.
